Welcome to the SPAYA 2023-2024 coaching application! Application must be completed prior to evaluations. Please be sure to complete all required information on the following form.
All on-ice coaches must register themselves with USA Hockey/pay $51 ($46 USA and $5 WAHA fee) www.usahockey.com
Coaches may request reimbursement for $51 USA Hockey Insurance fee, but must fill out a Reimbursement Form (found online under Coaching section), attach USA confirmation page as receipt, and turn into office for approval no later than December 31, 2023.
Mandatory background checks will need to be filled out through USA Hockey along with a $30 fee and is good for 2 years. Coaches may also request reimbursement for the background check fee.
